چکیده

Nine patients with different supraventricular tachyarrhythmias were treated orally with amiodarone (a Class 3 antiarrhythmic drug) according to a standardized protocol. Eight patients had subjective or objective beneficial antiarrhythmic effects from the drug. The treatment was discontinued in 2 patients due to corneal opacifications. In all patients right atrial monophasic action potentials were recorded with the bipolar suction electrode technique before treatment and in 8 patients after at least 4 weeks of treatment. The atrial monophasic action potential was prolonged in all patients after amiodarone treatment and increased on average from 234 msec to 308 msec. The findings demonstrate the specific mode of action of the drug and the use of monophasic action potential recordings to identify patients who, theoretically, could benefitfrom antiarrhythmic drugs whose action is to delay repolarization.

Amiodarone-induced postrepolarization refractoriness suppresses induction of ventricular fibrillation.

It is still incompletely understood why amiodarone is such a potent antiarrhythmic drug. We hypothesized that chronic amiodarone treatment produces postrepolarization refractoriness (PRR) without conduction slowing and that PRR modifies the induction of ventricular arrhythmias. In this study, the hearts of 15 amiodarone-pretreated (50 mg/kg p.o. for 6 weeks) rabbits and 13 controls were isolate...

Effects of intravenous amiodarone and ibutilide on action potential duration and atrial conduction kinetics in patients with persistent atrial fibrillation.

Class III antiarrhythmic drugs have been shown to be effective for termination of atrial fibrillation (AF). The aim of this study was to determine the steady state and non-steady state effects of amiodarone and ibutilide on the atrial monophasic action potential (MAP) duration (MAPD), effective refractory period (ERP), and intra-atrial conduction time (IACT) in human persistent AF.Fourteen pati...

Antiarrhythmic drugs: present and future.

The treatment of cardiac arrhythmias has undergone a sea change with the advent of catheter ablative procedures (radiofrequency ablation) and use of implantable cardioverter defibrillator (ICD). The antiarrhythmic drugs at times are used to prevent device related arrhythmia rather than being used for primary treatment of arrhythmias. Antiarrhythmic drugs are grouped according to their drug acti...

Neuroprotective effect of minocycline on PTZ-induced epileptiform activity and alterations of the shape of action potentials in snail neurons assessed by using intracellular recordings

Introduction: Epilepsy is a neurological disorder that affects 1-2% of the world population and about 30% of patients are resistant to antiepileptic drug therapy. Therefore, new treatment alternatives are needed. In the present study, the possible neuroprotective effect of minocycline against epileptiform activity induced by pentylenetetrazole (PTZ) was assessed. Methods: Conventional intra...

Chronic amiodarone evokes no torsade de pointes arrhythmias despite QT lengthening in an animal model of acquired long-QT syndrome.

BACKGROUND Amiodarone is an effective antiarrhythmic drug rarely associated with torsade de pointes arrhythmias (TdP). The noniodinated compound dronedarone could resemble amiodarone and be devoid of the adverse effects. In the dog with chronic complete atrioventricular (AV) block (CAVB) and acquired long-QT syndrome, the electrophysiological and proarrhythmic properties of the drugs were compa...
